Canadian plane crashes in KY
http://www.msnbc.msn.com/id/14540419/
after what has been called "the safest time in aviation" there hasn't been a major crash of a aircraft in North America; figures it was a Barbardie, and not a Boeing
somehow, one crew member survived of the 50 people on board

Possible operator error of some kind - either tower or cockpit, and they ended up trying to do 6,000ft of takeoff in 3,500...

Heard about this. Only the first officer survived, but with serious injury.

it was the air traffic controller with only 2 hours of sleep on a 15 hour shift. he/she had a 9 hour turn around between shifts and only had the 2 hours of sleep. and sent them down the wrong runway.

Pretty poor personnel management if that's the case. It's probably illegal too; in breach of occupational health and safety regulations. I see a lawsuit coming.
